2. X-Ray Fee of $42 Per X-ray

3. Extraction Fee of $380-$600

4. Lost Crown Replacement Fee of $220

5. Temporary Restoration of a Badly Broken Down Tooth $280

6. Re-Implant Avulsed Tooth Fee $380
